AUSTRALIAN ID CARD

The Australian ID card:

• Provides proof of age for Queensland residents aged 15 years or older.
• Serves as an ideal form of photo identification for those who are not old enough to obtain a driver’s license or do not hold a driver’s license or passport.

As of April 2019, the Australian ID card replaces the Adult Proof of Age card, which will no longer be issued. However, previously issued Adult Proof of Age cards remain valid until the expiry date shown on the card.

If you hold an Australian ID card and are applying for a car (class C) learner’s license for the first time, you may qualify for a reduced learner’s license fee.

The Adult Proof of Age card replaced the 18+ card. If you still hold an 18+ card, it remains valid.

How to Apply for or Replace a Photo Identification Card:

1. Apply for an Australian ID Card In-Person

You can apply in person for a photo identification card at a Transport and Motoring Customer Service Centre, a participating Queensland Government Agency Program Office (QGAP), or a police station that offers licensing services.

Requirements:

• Be at least 15 years old.
• Complete the Photo Identification Card Application (F4772).
• Provide evidence of identity.
• Pay the required fee.

Note that you may need to have your photo taken when applying, and your card will be sent to you by mail within 14 days.

When applying, you will have the option to include your residential address on the back of the card. You can add or remove an address on your card either online or in person.

2. Apply or Replace a Photo Identification Card—Living in a Remote Area

If you live in a remote area of Queensland and cannot visit a service center, you may be eligible to apply by mail. This option is also available if your card has been lost, stolen, damaged, or destroyed.

How to Apply by Mail:

Step 1: Download the Photo Identification Card Kit

The kit includes:

• Instructions.
• Photo Identification Card Application/Replacement Application (F4772).
• Specimen Signature Form.
• Guidelines for submitting your digital photo.

Step 2: Complete the Form

Fill out all sections of the application form. The form must be signed in the presence of an approved witness, such as:

• A medical practitioner.
• A police officer.
• A solicitor, barrister, or judge.
• Justice of the Peace or Commissioner for Declarations.
• Notary public or other authorized individuals.

Ensure your photos are signed and endorsed by the witness at the time of signing.

Step 3: Sign the Form

Print the form on A4 white paper, and use a black pen to complete all sections, including the signature boxes.

Step 4: Submit Photos

You must provide two identical color photos that meet the following requirements:

• Size: 35mm wide by 45mm long.
• Passport quality (photos from a photo booth are not acceptable).
• Printed on high-quality paper with a resolution of 600dpi or higher.
• Not digitally altered.
• Glasses or sunglasses must be removed.

The back of each photo must be signed and endorsed by your witness with the words: “I certify this is a true photograph of [applicant’s name] in my presence.”

Step 5: Provide Evidence of Identity

Include photocopies of your identity documents, signed and endorsed by an approved witness, with the words: “I have sighted the original document and certify this to be a true copy of the original.”

Step 6: Pay the Fee

Pay the fee by cheque or money order. Alternatively, the fee can be paid at a service center before submitting your application by mail.

Step 7: Submit Your Application

Review the checklist included in the kit to ensure your application is complete. This helps prevent delays in processing.
